The Perinatal Institute has developed documentation - Pregnancy Notes for the antenatal period, Birth Notes for the intrapartum period, and Postnatal Notes for the postnatal period. These are available for NHS Trusts and Independent Midwives to purchase.  This can be either as a complete documentation package or on an individual booklet basis.

The Notes can be purchased individually at £1 per booklet; alternatively the complete documentation package is available for £3.50. Additional complimentary items are included with all orders.

Diabetes in pregnancy have also been developed and are available for 75 pence per booklet including the clinic held summary sheet. In addition, Pre-conception Diabetes Notes were launched on the 18th of May and Postnatal Bereavement notes are currently being piloted and will be available later this year.

The use of the notes has been increasing steadily across the NHS. Currently around half the pregnancies in England are being managed with the Pregnancy Notes. See graph for uptake .
