The Perinatal Institute is engaged in a rolling programme of confidential enquiries commissioned by various NHS stakeholders, including WM SHA, PCTs and maternity units.

A number of projects are underway as a result of the findings of these enquiries. This includes a recently initiated, region wide project to develop and implement Standardised Clinical Outcome Reviews (SCOR), aimed to improve the understanding of causes and factors leading to adverse incidents, and to improve action plans and strategies for prevention.
